I'm sorry if this has been asked before, I tried searching and couldn't really find what I need.

I'm filling out my settlement visa application at the moment, and I'm at the part where it says "place of issue" and "issuing authority"

I know for issuing authority it is Department of state

place of issue, I would assume it would be USA, and on the passport itself it just says United States. But on the application, it specifies to put both the country AND city where it was issued. I have no clue what state it was issued though. I live in Indiana, I sent it to Pennsylvania when I applied for a new passport(because of my name change) and it was sent back to me from New Hampshire.  So do I still need to put a city and I'm just dumb and don't know which state to put, or is the application outdated and now we only put USA, and no city with it?  ???

Thanks for any help!

*edit* Also for my husbands UK passport, it wants the city/country as well. Under "code of issue" on his passport it just says GBR, lol. Authority I know is UKPA, but city? *sighs*

I was about to look at my passport for reference and forgot it's still at the Consulate  ::)

Really, there's nothing there?  Mine says issued from Connecticut.  Not that you didn't look carefully but maybe double check.  I know I can often easily over look things when they're important like when filling out that darn application!

Next to date of issue, there's a section on the passport called 'Authority'. That's the issuing agency. (In my case, it says 'Philadelphia Passport Agency' because that's where I got mine. Others will have different agencies/places listed.)

That's what you need to put down.

Yeah...I've been looking, I even had my mom look with me.

My old one says it was done at the New Orleans passport agency(I got that one in 2002) and this one in the same place as my old one just says:

Authority
United States
Department Of State


My old one says

 Authority
New Orleans
Passport Agency

I'm really at a loss! I really hate this new passport too...it looks fake. :/

Then that's what you put.

That's not a new thing. I've seen that on a lot of passports that were renewed by mail, back in the days when I was helping people with international travel for work.

In your case, the issuing agency is the DoS.

I know what the issuing authority is though, I said that in my original post. I need the place of issue, which on the application says to put the city and country. There is no city listed as a place of issue.  :-\\\\

I know my husband's didn't state a specific city either and it asked for it.  I think we assumed and put Belfast.  For yours, if there's space, could you just write in no city listed?  Or at the end when it asks if there are additional things you want them to consider, you could make a note there and say to reference your passport, and they'll be able to see for themselves there is in fact not one listed.  I made a note about something else in that section for the same kind of reason.

Yeah, no city is listed at all. So I will write a note at the end of it if I've not figured it out by then.

I'm not submitting it until tomorrow, just doing as much as I can until my husband gets back from a trip tomorrow so I an get his NIN. I figured I would have no issues with filling this thing out, but instead I got caught up on the second page. That's an expensive mistake to make though, so I just wanted to be sure of what to put.

Oh I totally understand!  That worry was in the back of our minds the entire time.  It's not like much guidance is really given, even in the "guidance notes."  Telling me to provide the information as given when I click on the question mark for more specifics is an excellent idea  :-\\\\

Try not to get too hung up on filling in the practice application in detail.  As we found out, it is actually different than the online one you actually have to submit.

EDIT:  Just thought you're probably doing the online one and saving it, so nevermind!  Good luck to you :)  Hopefully it will all be over for us and we'll be in the UK soon :)

Yep, this is the real deal!

And I agree about the explanations. When it asked my Moms name, it just says the family name for surname. I wasn't sure whether to put her married name or maiden name, the explanation wasn't exactly clear! I think when I look back though, once this process is done and dusted I'll see it wasn't really that confusing in the first place though.

And I think I recall it saying to put full names, even middle and patronymic, and elsewhere says to list it as shown in the person in question's passport.  The two could be completely different.  Probably getting hung up on things that don't really make any difference to them...but hard to not stress when it could be a $1000+ mistake as you say.

I've just tried to rewrite my answers to where it asks when/where/length/why I have been out if the country in the last 10 years. 10 years is a long time, I can't fit it all, they don't leave enough room. My comments at the end of this thing are going to end up a mile long! I guess they don't expect people to travel much? *sighs*

mine didn't have a state listed on it, I put united states.... cause that's what it says, cause it wasn't issued in oregon they ship it our or something, don't think it's done in state.

Yeah I just answered 'USA' and 'USA' in both of those questions.. They will have the passport there so there will be no problems.  :)

My husband's passport had been renewed in the United States so for his I put Place of Issue "USA", and Authority "GBR".  I sent my husband's passport in with my application so again they had it there to look at it if they had any questions.
11 Aug 00 - Married (Living in US)
25 Jul 08 - Spousal Visa w/ KOL Stamp Approved
01 Oct 08 - Arrived in the UK
05 Mar 09 - ILR Approved (In Person Appointment ~ Liverpool Office)
