reporting foreign accounts - FBAR TDF 90-22.1

Does anyone have experience with filing the FBAR TDF 90-22.1 form ?

We've just realised that my wife should have been filing these and hasn't. Does anyone have any advice for late filing and how to word an accompanying letter to explain this.
