Photos of forms needed to import pets into the UK

I've taken photos of my EU 998, Rabies Certificate, FAVN results (from the Kansas lab), the actual checklist used by the pet folks at Manchester Airport, and accompanying letter from my vet to cover my bases.  Hopefully folks can use these to know a) what they need and b) how they need to be completed.  You do not need a Facebook account to view these photos; if you would like a high res version please feel free to PM me.

The only form I can't find is my APHIS 7001, which I think was kept by Continental Airlines because they required it.  If I find a copy I'll add it to this post.

Thank you so much, I have been trying to figure out all those forms and instructions for a while now.  We are finally taking the animals to the Vet this week, and since this is the first time for the Vet and us, it was getting confusing.

mirrajay--mine had the sticker.  It's white and it blends in.  Maybe they just didn't notice?

Yes.  You must have the original.  My vet initially gave me a copy and intended to keep the original in my cats files until I explained that the original was required, so they kept the photocopy.

Could you contact the regional USDA vet for your area and have him detail the requirements to your vet (or vice versa)?  After all, IIRC, s/he needs the original in order to sign the 998.

I asked the front desk guy to have the one vet who is certified by the USDA to call me back.  I'm not holding my breath, but I will keep stalking him until I speak to him!

For anyone's future reference:  I learned today that the ORIGINAL titer result form that you will need is mailed to your vet, but sometimes not until a few days after they call you with results, so they probably wont have it when they relay your results to you.  My vet apparently doesnt know this as they told me that they only recieve it via fax. 
I called the lab in Kansas and was told that if they lose it (which is possible knowing them) then a replacement original can be obtained for $34 (straight from my vet's pocket if needed!).  But its good to know that the original can be duplicated with sticker and everything, just in case.

I just wanted to check about the definition of ORIGINAL titer result form - I know you have to have the one with the sticker, but is that the actual submission form that the vet fills in and sends off, of do Kansas keep that one and send you a copy of it, with a sticker on? It's just that I read on the Kansas website that:

"A FAVN Submission Form is required for each sample submitted for rabies antibody titers for
export animals. The original submission form is retained on file at the K-State Rabies
Laboratory.  The rabies antibody titer results are placed on an adhesive sticker on a copy of the FAVN Submission Form – this is the official FAVN report".

Just wanted to be sure on this!
