Yes, your vets office fills it out (preferably in blue ink) and sends it to the lab requesting that the test be run.  They put a sticker on the bottom of that same form and send that paper back to your vet.  THAT is what you need.

I just wanted to check about the definition of ORIGINAL titer result form - I know you have to have the one with the sticker, but is that the actual submission form that the vet fills in and sends off, of do Kansas keep that one and send you a copy of it, with a sticker on? It's just that I read on the Kansas website that:

"A FAVN Submission Form is required for each sample submitted for rabies antibody titers for
export animals. The original submission form is retained on file at the K-State Rabies
Laboratory.  The rabies antibody titer results are placed on an adhesive sticker on a copy of the FAVN Submission Form – this is the official FAVN report".

Just wanted to be sure on this!



This is correct -- Kansas does keep for their files the FAVN form your vet sends with the blood sample, and they send back to your vet a copy they make of that form on which they have placed the sticker.

Exjerseygirl, you're simply awesome!  Those photos help to make this whole process so much more clear - most especially for me the EC998 forms.  I wanted to thank you for spending so much time to help us out.  Your posts have given me the courage to start this whole messy process.  And, I'm happy to say that I've gotten off to a great start.  My pup has been microchipped, vaccinated and her rabies titer has come back from Kansas with a sufficient level of antibodies.  I'm so happy!!! :D  We're currently planning a late October flight for her for her UK entry.  I'll let everyone know how it goes for us.

This is a question regarding the EU 998 form.  This will be the next piece of paperwork that I need to fill out
I looked at exjerseygirl's forms, and I have just a few basic questions about who and when will fill out the paperwork.

Based on the photo of the EU 998 form, it looks like the pets regular vet fills out the first page of the form to confirm all information about the animal and the titer test.
The second page of the form (top portion) looks like this is the only area that is validated by the USDA vet.  It will be sent to him after the first portion has been filled out by the regular vet
Below the USDA portion on page 2, this looks like the area where the animals regular vet confirms the flea/tick/tapeworm treatments shortly before departure.

This form looks like a 3 step process form.  Is this correct ?

Also, when would be the best time to get this form filled out ?  Based on the date of their blood draw, we should be good for travel on 10-24-2009

I believe the information you listed is correct, artistplace, but I haven't gotten to the EC 998 form yet.  I do know that the EC 998 form is valid for travel within 4 months, so probably the best time to get it done is late August, in case the USDA vet is slow to process things...that would still give you until December to travel, in case your plans change as well.

I filled out the various sections as soon as I had the information. For instance the name and address for the vet (not the signature) I filled out sections I, II, III, V myself as soon as I started my cat on the process. My vet filled out section IV. I also filled out the address portion of section VI and VII. My vet signed section V a few months after I got the rabies test results back but you can have him sign that part anytime after the test. I sent the form off to the USDA about 2.5 months before we are leaving. I have a fixed travel date so I wasn't worried about having the form signed too soon. Once you know your travel date you can have the USDA sign off on it up to 4 months before leaving. The USDA will fill in the serial number of the certificate (on page 1).

Not sure if this really answers your question...
01 March 2009: online application
13 March 2009: biometrics
16 March 2009: submitted documents via FedEx
18 March 2009: Tier 1 Approved (NY consulate)
18 June 2009: Tier 1 start date
29 August 2009: Arrive in London
22 April 2010: Finally got a job

Sorry, one more question here... Where do you get the APHIS form?  Should my vet have this?  And what aout the customs forms? Are those given at the airport?

The APHIS form can be provided by the USDA vet, though the vet here in NC said it wasn't necessary.  (If it turns out to be required for you, it's a form in triplicate, which is why it's not available for download.)

Not sure about customs forms, I assume those would either be at the airport or possibly even on the plane with the other customs declaration stuff.

The information from everyone is excellent, and just what I was looking for.  Thank you so much for the timeline as to when this paperwork needs to be filled out and submitted, and the steps involved.  My animals had their blood draw on 4-24 so I would assume that they can travel on 10-24.
My husband is on a 5 year work assignment in the Cambridge area, and I plan on joining him within days after my four legged kids are cleared to go.
Gotta fly BA out of Dulles, and I would like to do this by 10-25

Another bit of good news.....I just sold my house the other day after only being on the market for 10 days. Final closing of the sale will take place on 7-28.   What a shocker for me.  The good people that are buying it are also going to let me rent it from them until the first of November (four legged kids will still have their yard). 

The animals and the house were a bit of a stress, but as of now, it all looks good so far.
All of you have helped me out immensly with the PETS process, and have really lifted the stress of getting the animals over there.  Thank you

   I'm on the final countdown, but was given some misinformation from a pet shipper I was thinking of using...so, I'm three weeks out and have to get the EU998 filled out still. I was told it had to be within 10 days of departure...I've just booked my ticket and now the running around begins! Luckily, I only live about 90min away from Harrisburg, PA where my USDA vet is--so I made an appointment to get stuff signed/stamped for next week.

I had this question, too!!  I was so confused, but the USDA was awesomely helpful when I called (possibly flipping out a bit).  

Apparently some forms that are pre-printed (not printed off the internet) already have a serial number.  Some vets have them, but most do not.  If you (or your vet) printed the form off the website, the area is blank and the state USDA vet will assign a serial number to your form when they endorse it.

The APHIS form contains health information about your pets not included on the EU998 form.  Your airline may require this form to validate your pets' ability to fly safely (Continental required it in my case) or they may simply accept a statement of health from your vet.  If your airline requires it, it must be completed no earlier than 10 days before your departure date and signed by the USDA vet.

In my case, the USDA vet used the number on the APHIS 7001 form as the serial number to show that the two forms were for the same pets/travel, etc.
