Found it. It's on a DRAFT proposal from the IRS site. It's at the bottom of page 4. A good deal of new information is being made available, probably as a result of the Taxpayers Advocate report,

Thank you.  I will definitely call that number after 90 days from when I sent in my 2009 FBAR.  A free phone call makes a lot more sense than paying to send the form by means other than regular mail.
