Proof of Immunization question...

David went to his GP to get his immunization records - and they wrote it down on a piece of scrap paper, put it in an envelope and mailed it to us. Not very official, eh? So I went back and complained, and all they did was photocopy said hand-written note onto letterhead. SIGH. So... what kind of proof do they need at the medical? The note says he's received his measles, polio, dip, & tetanus jabs - are those the only ones we need to prove?
I'm done moving. Unrepatriated back to the UK, here for good!

no he may need more... i have the list i found online ... it is as follows:
mumps, measles, rubella, polio, tetnus and diptheria toxoids, pertusis, influenza type B (Hib), hepatitus B, varicella, pnuemococcal and influenza
 now I may be wrong but if you go to the us immigration website it will tell you what records you need... thats where i found my list... Hope this helps

no worries - Martin's "record" was only on a scrap of letterheaded note paper too. hand written no less. it passed just fine.

dont worry about the flu shot - thats only for older people. varicella - not needed in UK

he's had dip and tet - check
polio - check

mmr - well he's had measules shot. has he had rubella or mumps? if so I think he'll be immune now. if not - he can have seperate jabs or your doc can write a note saying they dont think its medically appropriate. ask your doc if having the mmr after already having a measules shot is ok.

pertusis & hep b - not sure if mart had these. I'll check. you can always ask his doc if they think theyre medically appropraite and get them on the NHS if they think its ok. lemme check marts requirements tho.

Hi there,

All you need is Dip and tetanus and MMR. they have changed it recently, original form I had said polio as well but the new one that came with the packet 3 from the embassy for my interview said that polio was only needed if you were under 17years old.

The health authority lost my medical records when I moved!! My doctor said it was no problem having dip/tet again even if I'd already had it. He didn't reccomend having MMR as I'd had mumps and measles and rubella as a child...lucky me ;)... so they just did a blood test which confirmed I had the immunity.

Mine was also hand written and it was no problem  :)

As long as it is on some letterhead paper that's fine!
So my choice is "Or Death"?

Cool, thanks. :) I suppose what we could do is schedule a blood test for him to check that he's got all the right immunities, and then bring that? He doesn't haveproof of his MMR - - - they've lost his records from a child as well. (Rolls her eyes so hard her skull almost cracks).

I understand completely... It's amazing how they can just lose you medical records! 

That is exactly what I did and they were fine with that.  :)  All I had were the printouts confirming I had the antibodies.

It took a couple of weeks to get the results back, so make sure you schedule the blood test as soon as you can.
So my choice is "Or Death"?
