but since i have the appointment all set, i need to triple check something:

My then-husband can't apply with me for the spousal visa.  His work made him return to the office, leaving me to meet him with my visa.  Since i won't have his passport, i want to triple check that if i get a copy of his passport notarized, they won't deny me. 

the ticket is bought for the 10th of September baby!  Don't turn me away!!!!!!   

Every page - even the blank ones - of his passport have to be certified/notarised.  But, yeah, that's acceptable.

also, when it asks for Mother's Family Name or Surname

is that her maiden name?

I believe I put my mom's married name....

omg, and am i writing this like i'm already married?  it asks for my status.  i'm applying as a married woman but as i write this i'm not married!

WTF.  this is so confusing!  lol.

duh, i should def. say that i'm married cause when i file i'll be married.

It's asking for the family name on your mother's birth certificate.
