multiple pets on 998 form?

I thought I read somewhere that you could list multiple pets on the 998 form and then only need to pay the USDA fee once.  Does anyone know if this is true, and how I would do that?  Thanks for your help!

I've never heard that, I believe every pet needs its own paperwork. DEFRA is most concerned about the microchip numbers matching all the vet records, so if both are listed on the same form, that would make it way more confusing to read.

FWIW, I paid a large fee to the USDA for the first pet ($100ish?) and a much smaller fee ($10 or so) for the second pet. If it were an option to list both animals on one sheet, they wouldn't offer different pricing for multiple animals.

You can call your USDA vet to find out for sure, but I'm 99% sure this wouldn't be allowed.

i was told it was a seperate form for each pet.....i think equestrianlover was in nc as i was.....i too paid a large amount for the first one then a very small amount for the second.....best of luck to you

Thanks - I'm not even sure where I thought I read that.  I did call my USDA office today, and they said individual forms, $114 for the first animal, $6.50 for each additional animal.  They also said that some airlines accept more than one animal on the International Certificate of Health, so maybe that's what I read!

In any case, I appreciate your replies. :-)
