is my DH UK birth certificate in long form needed?

I followed the instructions about ordering a new long form birth certificate.  It came today and it's just one sheet of paper with headings like, "child", "father", "mother" and "informant".  I'm wondering if this is the long form or another form?  little confused - I assumed long form meant is was long?

anyone have any direction to a link showing what a "long form" uk birth certificate looks like?

Long form just means A4-sized... it's still only one sheet of paper.

Basically, there are two versions of UK birth certificates: a short form, which is about half the size of an A4 sheet of paper with only the basic info on it (name, date of birth, parents names etc.) and long form which is full size A4 with more details on, such as details of the parents' places of birth as well - the long form one is needed for visas because it shows the full details.

My husbands 'short form' from 1969 doesn't even list any of his parents information. Neither does my daughters and it's from 2008. Just the name, date & place of birth. It's not even anything like what ksand linked to. His is just like this one: http://familyfolklore.files.wordpress.com/2011/11/bcertmcw.jpg

However, his long form is as you describe what you received.

Thanks :) It is the A4 form one.

Now I've just noticed one of his two middle names is spelled different than his passport and how he spells it.  His passport and everything else says "MaThew" but noticed the birth certificate says "MaTThew".  Wondering if I should be worried about it.

visajourney.com is a really helpful website for US visas.. I don't know if you're familiar with it?
