HMRC forms to claim DT relief for 401k

If you have a 401k in the US; are not taking income from it yet and are a UK resident, what HMRC form do you use to declare it and claim relief from UK tax on any gains under the US/UK double taxation (DT) agreement

I never reported it to HMRC until I got a distribution, i.e, I didn't report it for about 12 years.

I believe that strictly you are supposed to claim DT exemption from UK tax on 401k gains by communicating with HMRC. I can't find a form that is specific to this so when the time comes I'll probably just write something in the "Additional Information" bit of the SA100 and won't include the 401k on the SA106 until I take income from it.

You can put it in a white space note on the SA100 - typically the employment pages but it doesnt really matter.  There is no separate form.
