applying for uk visit visa help with approved time off letter

i work as a caregiver, and i'm going to be applying for a visitor visa and my employer doesn't know exactly what my time off approval letter should include that will be efficient enough.

does anyone have any rough drafts? or main points aside from the dates that i will be visiting should be? or anything else it should include? i'll be visiting roughly under a month.

I've carried the following, signed and on company letterhead, whenever I've traveled, in case immigration ask for it. Caveat: I can't vouch for whether it will work when you're applying for a visitor visa in advance of landing. It might at least give you a start, though.
