If she gets antibiotics an ear infection should clear up fairly quickly and she might be a bit uncomfortable on the flight monday on landing and takeoff, but it really should clear up right away!
As far as travel insurance, as long as it is emergency care in A&E, (I just learned this by the way)!

make sure you keep the paperwork as the question is asked on the spouse visa if you received health care in the UK and if you have an NHS number, which, if she isn't admitted, she won't get. I am not sure if this even applies to children or not though? But it should be nothing for you to worry about, so do what you have to do, if you feel she needs to go to the emergency room, take her there to get checked out.
