Vet form needed for travel

Old regulations required an EU form 998 and US vet health certificate for travel.

Do I still need both forms? I'm assuming not base on my research but wanted to double check.  I've got the specific forms Virgin needs and can get health certificate from vet and stamp from state office.  Just wondering am I missing something? I'm not seeing any mention of the EU form anymore like I did when my pets flew to the UK in 09.

I'm in GA but flying out if FL hope it's not an issue I don't have a FL vet.  Wouldn't think so but last minute worry is setting in.
