Where to find Annex 4?

Every one I have found has lines through a large part of it.  I know I saw a clean version of it somewhere.  Are the vets suppose to have this document or is it something I need to take to them?

I believe this is because there are two versions of the form commercial and non-commerical. The form is identical but not all boxes are needed. Here you have the non-commerical form which requires less information.

We moved in March of 2013 so I am not up to date with the latest regulations but I did manage to dig up the old paperwork.

What I have is an Annex II but it looks pretty much identical to the pic you posted, diagonal lines and all.

When doing your research, your two main points of contact  should be your local USDA office and the animal reception center of the airport at which your animal will arrive.

You should speak with your airline too obviously but you will get more sense from the USDA/ARC.

You should be prepared yourself with the forms and be familiar with the procedure, don't rely on your vet knowing what to do.
