I don't understand why this would happen, any insight? If applications a and b are put in a priority pile based on when they are received, how can the one lower in the stack get done first?
Makes it sound like it's totally random and if you are in the priority pile it is just luck of the draw and not based on first come first serve. Or am I missing something?

There is no real rhyme or reason to it - each application is different and will not necessarily take the same amount of time to process as another application.
It could be for a number of reasons. For example:
- it’s likely that the ECO’s are each assigned a pile of applications... and it may be that one ECO works through their pile faster than another ECO
- an ECO may go off sick and so their applications would have to be reassigned to another one
- one person’s application may be more complicated than another, so it will take longer to make the decision
- if more documents need to be requested, that can take longer as well
