He says thank you and wants to know if you think there's any way I can bring note to the discrepancy on his UK visa refusal document while staying neutral, or if he should let it be entirely and just provide information.

Yes, absolutely neutral. Just state the facts as jfkimberley says. Include everything up front and make it as easy as possible for them to figure out what happened for themselves. :)

I currently have 80 pages of evidence that I'm getting ready to ship off to the hubby - want to check that this seems normal or if anything pages can be cut out.

I've included my entire tenancy agreement because it's signed at the front and back.
I've provided every single page of my bank statement each month - he wonders if I can just include the page that my wage appears or if I need the entire statement,

Cards - 8 pages (1 card per page)
Facebook - 2 pages
Hangouts - 2 pages
Payslips - 7 pages
Photos - 1 page
Work contract - 9 pages

80 pages is reasonable.  As most bank statements, tenancy agreements, and contracts are long, seems right.

You need ALL the pages.  Are you scanning or posting?  If posting, do NOT use Royal Mail or ParcelForce.

- 2 photos - beginning of relationship and wedding

- chat logs: (skype delete A LOT of our chat history so we only have a small amount from 5 years of conversation)

What are the dates on your payslips?  Do the pay dates span six FULL months?
9/1/2013 - "fiancée" (marriage) visa issued
4/6/2013 - married (certificate issued same-day)
5/6/2013 - FLR(M)#1 in person -- approved!
8/1/2016 - FLR(M)#2 by post -- approved!
8/5/2018 - ILR in person -- approved!
22/11/2018 - Citizenship (online, with NDRS+JCAP) -- approved!
14/12/2018 - I became a British citizen.  :)

Oops, so you do.  I miscounted!  But better to be safe, right?

I don't see anything missing from your end, other than the signed declaration, which you've noted and will send separately.  Just remember your most recent bank statement, payslip, and employer letter must all be dated within 28 days of the application date, so don't dawdle now that you know the dates of those documents.

Okay, the main reasons he was refused were:
- not enough money to support himself without working for 5.5 months
- not enough evidence that he was a genuine visitor in the UK (evidence of plans to sightsee etc) and that he had ties to the US (a job and a home) to return to.

The part about Australian immigration was just an added extra that they stuck in to back up the rest, and is of little consequence, as he would still have been refused even without the Australian visa issue.

So, for the fiancé visa, there is nothing to address, other than stating he has a refusal and including the paperwork, because you are now proving the opposite of what he had to prove for a visitor visa. You are showing that the reasons for his refusal are now the reasons for applying for the fiancé visa.

For a visitor visa he has to show:
- no intention of living in the UK
- he has a life to return to in the US

But for a fiancé visa he has to show:
- he intends to live and work in the UK permanently
- he is leaving his life in the US to make a new life in the U.K.

So, none of the main reasons for his refusal apply anymore.

Nope, makes no difference in regards to the refusal or the cover letter.

The cover letter should only state:
- visa type applied for
- date he wants the visa to start
- details and explanation of his refusal  (I.e. refused due to not having enough money available for a visit or evidence of tourist plans for visit).

I'm looking at the VAF4A, question 3.12 asks have you been earning the amount, detailed in 3.11, continuously for 6 months prior to the date of application?

How do I answer this.

Explanation - so from August 2018 I was earning 18,888 per year, in February the company and our union decided we should all get a payrise. My April 2019 payslip shows the new annual salary of 18982.08 which was backdated to August 2018.

So i've been earning the income requirement for more than enough time and my employer has explained this in the letter, I just want to know if I put the new salary or the older one.

Tick yes ... because really, it’s asking: “Have you been earning more than £18,600 continuously for 6 months?”

And put your new salary in the box.

If you are salaried they will take the lowest payslip from the 6 months and multiply it by 12 to get the total annual income that can be used for the visa.

As long as this lowest payslip is at least £1,550 before tax, you will meet the requirement.

If you were to tick No to that question, then you would be saying you don’t qualify under Category A and therefore you would have to use Category B and would need to provide 12 months of payslips and bank statements.
