FLR(M) date calculation and application

Sorry to clog up the lines, but could someone point me to the thread that discusses how to figure out when the earliest date to apply for FLR (M) is based on first UK entry (on spouse visa)? I've seen it before, but now can't find it anywhere. I still have about a year to go, but planning travel and whatnots for next summer, so need to work this out!
Also, is there a list somewhere of documents to provide? Based on the guidance, it is about the same as the first round, but after that fiasco (hellllloooo five month wait), I don't trust my own interpretations!!

If you have a spousal visa, the earliest you can apply for FLR(M) is 28 days before you reach 30 months (2.5 years) in the UK.

So you count 30 months from the date you entered the U.K. (the date on your visa stamp) and then take 28 days off that.
