Unless you have serious criminal convictions or bad enough UK immigration history that means you are not eligible to enter the UK, they can only refuse the visa if you either:
- don't meet the financial, accommodation or relationship requirements
or
- don't send the required financial, accommodation or relationship documents
1. Can they really refuse a visa for the wrong coloured ink?
No, they cannot.
2. Can they refuse if documents aren't in order. There is pretty much zero guidance on what has to go where at the minute and I've obviously done the best I can.
No they cannot. It would be extremely hypocritical if they could, considering they don't tell you the order until AFTER you have already mailed everything to them.
3. In our packet, we have printouts of our facebook chat, 1 page per month that we have been in contact clearly showing the time, date and the names of sender/receiver. We also have a love letter each that we wrote each other and also a couple of birthday cards sent. Is this enough? I really don't want to be refused for inadequate relationship evidence.
Do you have boarding passes/e-tickets for flights taken to visit each other as well? Those will carry more weight than the messages and cards.
Otherwise, the rest sounds fine to me. In fact, you probably have too much for the Facebook evidence. You just want 2-3 pages in total, showing messages from ALL months. No message content needs to be included, just the date, name and time.

4. We have included 2 passport photos of the applicant. I didn't however provide one of me (the sponsor) major panic. Do I really need one??
No you do not need one. And in fact, you only need to send 1 passport photo of the applicant... which will not be used and will likely be returned to you anyway.
You are mixing up the passport photo requirements for a spousal visa with those for an in-country FLR(M) visa.
Spousal visa = 1 passport photo of the applicant, no passport photos of the sponsor
FLR(M) visa extension from inside the UK = 2 passport photos of the applicant with their name on the back, 1 passport photo of the sponsor with their name on the back
