The ILR form and about Section 1 and 2

Okay, I have a question. In Section 2, which is about me, the applicant.

1) Do I put my name as it is on my passport(e.g.) Jane Ann Doe and not my current married last name?

2)  And where would I put my current last name. I'm figuring that if not in the name section, it would be under the section that asks for other known names.

3) And in Section 1, what is 1.4 "Home Office Reference."

I put Lola Ellen Maiden, as the name in my passport hasn't been changed.  My ILR was issued in my maiden name.

If you have a home office reference number, you'll know it.   ;)

Okay, that went right over my head. Is it something you get when you call for an appointment? Or wha? ???

No, really -- if you have one, you'll know it.  If you don't know that you have one, you don't have one.  Don't worry about it.   ;) ;)
