US and UK Taxes

I have worked in the UK since October 2005 and plan to leave in May 2006.  From October 2005 till December 2006, I have earned less than 4000 pounds.  So here are my questions..

1) Will I have to pay taxes in the USA?
2)Can I claim back any taxes I paid here in the UK?
3)When filing taxes does one provide pay slips from wages earned in the UK since the tax year in the UK ends in April when the US taxes are due.
4)Which form does one use for an extension of US taxes?

Also, does anyone know of an accountant in the UK that does free consultation for international taxes.
