Is this something I should worry about?

My dog's EC 998/2003 form is two pages, although his name and microchip number is typed on the second page.  Also the USDA vet wrote the certificate number on both pages.

Is it a problem if it is two pages??

Also, my vet isn't very smart about this whole ordeal and I'm wondering what tick treatment your vet gave your dog...

2 pages is fine!

I have a cat, so can't help you on the tick treatment  :)
