Another Question about the FAVN-OIN Rabies Test

I know there are a lot of threads already discussing the process but I'm on information overload. I hope someone doesn't mind answering this even if it's been covered before.
I'm taking my dog in tomorrow morning to get this test done.
Do I need to bring any paperwork to the vet other than the "Request for FAVN-OIE Rabies Antibody Test" form? I also printed out the directions.
