pets passport

Hi can you help me.  I have had my five (yes 5) animals micro chipped, rabies shot and blood work done, but all i got was the report from the lab showing the antibody levels in the blood.  What do i do now to get the pets passport - is it more paperwork?  My vet is just a stumped as i am - We are due to move at the beginning of Feb.   ;D
