Help with these forms!

There is a lot of talk in the PETS scheme thing about a “vaccination record.” Is this an official document or just an informal record? I have a list from our dog’s vet chart, but it’s a mess (my name changed, she has two microchips, she got double vaccinated, half is from a humane society, and half from our "accredited" vet, etc.)

I am also slightly confused about the two forms. We are at the point where our dog has been microchipped, vaccinated, and her blood has been sent to the lab. I think I am clear on the forms needed (one we've printed off, one comes from the vet, correct?), but not exactly WHEN they need to be filled out. We will be leaving before our dog does (us in October, our dog in February). I feel like having our vet fill out, sign, and date APHIS and 998 when I won’t be taking it to the USDA place until February, is going to cause problems.

Can I have the vet fill out parts of the forms and then I can hold them until necessary when I come back to pick up our dog? So far he hasn’t filled out anything on either of the forms. Or, can I have him fill them out when we get the blood test results back, and they will still be perfectly valid in Feb??

I am so concerned that I will screw something up when we are already in the middle of the process. Any help greatly appreciated.

If I remember correctly, the Aphis 7001 form does contain the animal's vaccination information, so this could be what they are referring to.

In Georgia (this year anyway), it was within four months prior to the date of travel.

Call the USDA in your state and ask them.
