Am I good to go?

So we moving both of our kids, dogs and have done everything we need so far...I just want to make sure I am not missing something....

We did microchipping for both.
New Rabie shots for both.
Titre tests came back good for both.

Now what else do I need?  Is there some other form besides the FAVN-ONE that has the Titre test results on it?  I know I need to get something endorsed by the USDA 10 days before they leave....is this the Test rusult page, or does my vet issue me something?  If so, what is it and when can I get it from them?

Best for you to call the USDA directly and then your airline to find out what you need exactly.  It's different by state.

The USDA form 98 2003 is available as a PDF file.  The APHIS 7000 is in triplicate, so not downloadable (I think).

If you enter PETS in the search box, you will be directed to good information.  In the end, it depends on the current USDA/airline requirements.

Is the APHIS 7000 something that my vet should have?

As for the airlines...we are using a pet moving company....

A USDA Accredited vet should have the APHIS 7001 form however.. in my searching I have found that not all of them have these.  My local USDA office (in Austin) explained to me that the vets are accountable for those forms so not all of them order them.  However if they are accredited with the USDA and they want the form they just need to order it thru the USDA directly and they told me that takes a couple of weeks for them to get to the Vet's office. 

Keep in mind that the Pets Scheme itself does not require the APHIS 7001 form, this is something that if required is required by the airlines themselves prior to departure.  This form is telling the airline that within 10 days of travel the pet was seen by a vet and they are 'safe to travel'.  Some airlines require the APHIS 7001 form and require it to be stamped by the USDA, some airlines require the AHPHIS 7001 form but do not require the stamp from the USDA and some airlines will simply accept a written out letter from a vet saying the pet was checked out and is okay to fly.. it is all up the airline that you are flying with.

Hmmm....so best bet is to talk with the pet moving company and see what they need...I would think they should know best...i hope!

Yeah when I was going around and round with BMI they kept telling me to contact the agency that will be clearing the animal in the UK.  When I contacted Pets on Jets in Manchester they told me flat out that the Pets Scheme requires no such paperwork and anything related to a 'Health Certificate/APHIS 7001' would be something that the airline directly requires.  All they will be checking upon landing in Manchester is that the 998 form is filled out appropriately, stamped by the USDA Office, has the Tick/Tapeworm treatment within the specified timeframe, rabies certificate (that shows the animals microchip number) an the original Titre result w/ the raised sticker.

Also Pets on Jets let me scan & E-mail my paperwork to date so that they could review and they let me know everything is spot on and confirmed that the tick/tapeworm treatment my vet planned on using was approved.  They have been a GREAT help to me!
