health insurance while in the US

Hello.  I have a long-winded question.  It may belong in another forum topic; if so, just let me know.

I am a US citizen (though I've just applied for UK citizenship).  My wife is a UK citizen.  My kids both have dual nationality.  We all live in the UK.  I work for a UK company.  My wife runs her own UK business (as a sole trader).

For personal reasons, it is very likely that we will be moving back to the US (California).  We don't know yet if this is a short-term or a permanent move.

My wife will keep some clients and continue to work from the US.  In time she plans on setting up shop there and growing the company as a US business.  Of course, that will have to wait until she has a green card. 

I have spoken to my employer and they have, amazingly, proposed that I could work abroad for them.  This has pros and cons, but increasingly it is appealing to us because 1) it's proving difficult for me to find a job in the US, 2) it would allow us to look for a home anywhere, and 3) having both of us working from home would help out with childcare (and presumably "quality of life") enormously.  One concern that I have is with the abysmal US healthcare system.

Currently I have National Insurance taken out of my paycheck to fund the NHS, which I am entitled to use for free.  No such system exists in the US.  So, would I be responsible for paying for our own private health insurance?  If so, would I still have National Insurance payments taken out of my paycheck?  If not, how do UK employees (and their families) get healthcare in the US... without going broke?
