New 3rd country cert

Has anyone had a look at this?  I had a look on the Defra site and its all there but it looks confusing to me.  Which part are we supposed to use and does anyone have an example of a filled in one?  It's just for 1 dog.

A friend of ours will be doing all the paperwork for us and I want to make it as easy on her as possible.  Also because it is new I would like her to be able to make sure the Vet does it right.

If you can give a link to it, some of us can take a look.  I'm sure they havent change TOO much about it.  All rules (blue ink is best, must use European date format, etc) have likely stayed the same.

I also called Manchetser animal reception yesterday and the guy there told me we could still use the old one for a few more months since they are still getting used to the new one themselves.  However he said the airline or USDA vet might want the new one.  I was thinking of having her do both just as a backup

An example would be great if anyone could do it

Depends on when you are moving - I believe DEFRA said the old ones will be phased out by the end of Feb, meaning you must have the old one signed before the end of Feb for it to be able to be used.

I had a look over it myself (am moving my cat this Oct) and I'll be calling my USDA vet in a few months with some questions but if I read right, the ANNEX II is for ONE pet, at the top in the right corner you will see it say something like 'for five or more dogs, cats, and parrots' and the one you need will only say one.

However yes, if someone can post a done one or something it would be great. I had the old one printed out and hte format is completely different...

The plan is to have to dog move on March 7th and I was aware of using having to use the new form.  I was actually surprised when the guy at animal reception said that they would accept the old one.  He said so far they had only seen 1 completed new form and I think are just being understanding of our situation.  I am planning on getting the new form done and maybe just bring an old one as a backup incase it gets filled out wrong.

Thanks for the help but yeah if anyone has a completed one they'd like to share that is always helpful
