Urgent! Where does local vet sign off on Annex II for tapeworm treatment??

I'm losing my mind. I cannot figure out for the life of me where my local vet is supposed to sign off on the tapeworm treatment section of Annex II. Can anyone tell me specifically where he signs??

From what I understand, you're talking about page 2 of Annex II, section 11.6.
The further most right box says Administering vet (name in capitals, stamp, signature); that would be my guess. What is most important is that the details are there and correct AND signed off on. Even if the stamp/signature and name is written in the margin next to the information, I think they'll take it.
