I'm assuming you mean the UKBA visa application (i.e. the VAF4a or the online application), because I have never heard of a Worldbridge application.
Worldbridge have nothing to do with the visa application itself... they are just a commercial partner of UKBA and all they do is offer the priority service on their website, which you can purchase after you have applied online and attended the biometrics appointment.
I don't remember the question being on there either when I did the start of the application in April last year.
There is a new stipulation that you cannot have more than £1,000 in outstanding NHS charges when you apply for a UK visa, or your visa will be refused.... which is probably why it is on the application now, but it wasn't a few months ago - they need to make sure you haven't used the NHS as a visitor without paying for it.
Remember that the application form and the fiance visa requirements have changed since you first started the application last April, so there may be several things on the form that are different from last April... for example, all the accommodation and financial details must now be entered in Appendix 2 instead of the online application.
