Need help with US + UK baby passport form asap....

Sorry about the "asap" i am having trouble filling out the US baby passport form online and i need the answer today as my appointment is on monday.

On the form about the baby's parents, i have fillied in the baby's mother and fathers information but underneath it there is a spouse section, is that referring to the parents? if i select yes it asks me for spouse's information and "Date Of Most Recent Marriage" but it doesn't accept the date of my marriage for some reason, any advice?? How do i fill out this information?

I am not sure what to fill out on the next page as i can not continue on the form as i did not complete this part of the form so is there anything else i need to know to fill in this form?

Hi,
Your baby is the applicant, so answer all the questions as if s/he is filling it out. So, no, the baby has never been married.

I got the photo's taken today but baby eyes was close and head look a bit big on both UK/ US size photo.  Worry it not accepted... :-\\\\

It's been months since I last got baby passport, forgot how it was done...

Do I need co-sign on UK passport photo, 2 photo, do I need to write date as well?

I don't need to get US photo sign or anything?

 ???

Nope, one photo of baby is all they ask for. Not countersigned like the UK application. And under a year old, eyes closed is acceptable. If you have a small ruler lying around, you can actually measure baby's head in the photo and just compare it to the acceptable size. (sorry I am on my ipad and have no idea how, or I would find you that info and link it.)

If you have spare pictures, take them along anyway.

On the US report birth form, number 24, what do I write?  The last time I been in US?  How much do I list?

They want to know all of the time you were in the US. For example, I wrote the following

"Birth - Sept 19XX, resident of State of X.
Oct 19XX - Jan 20XX, resident of State of X.
Jan 20XX - Nov 20XX, resident of State of X.
Nov 20XX - relocated to country of X."

(By the way, I wasn't sure what they wanted there either, so I left that one blank and asked the officer when I got there, and that is how he instructed me to fill it in.)

Then I included a list of ALL of my international travel outside of the US, typed on a separate sheet of paper.

Someone please correct me if I am wrong.. But I was under the impression that for US passport the babies eyes must be open. The shop I took my daughter to did them with the eyes closed the first round. I returned showing them the US passport requirements and they redid the photos for that one!

from the Passport photo requirements page of the US Embassy website:

All photographs must taken within the last 6 months and be 2" by 2" (two inches by two inches), excluding any borders;
Make sure the photo presents the full head from top of hair to bottom of chin; height of head should measure 1 inch to 1-3/8 inches (25mm to 35mm);
Eye height must be between 1-1/8 inches to 1-3/8 inches (28mm to 35 mm) from the bottom of the photo;
Photograph background must be white or off-white;
Applicant image must be centered and facing forward;
Photographs must be taken in color (NOT black and white);
Position subject and lighting so that there are no distracting shadows on the face or background;
Be taken in normal street attire, without a hat, head covering, or dark glasses unless a signed doctor's statement is submitted verifying the item is used daily for medical purposes.

*In the case of a baby, the eyes are not required to be open. *

Great if that isn't a requirement.. However, I went by this myself:

No. Please have your child’s eyes open and looking straight ahead towards the camera.

What’s the best way to take a photo of a baby or toddler?
When taking a photo of your baby or toddler, no other person should be in the photo, and your child should be looking at the camera with his or her eyes open."

luckily, they also had a photo booth onsite in case you get it wrong (dec 13)!
January 2000: meet sweets
August 2000-August 2002: LDR
September 2002-August 2004: student visa in the UK
September 2004: student visa soon expiring; move to NYC
November 2004: sweets decides he can't live without me and moves to NYC
January 2005: Married!
July 2012 (6th): submit (initial) application + payment
July 2012 (27th): biometrics taken (b/f appt)
August 2012 (3rd): send HUGE-almost-laughable-in-size visa application
August 2012 (7th): receive confirmation of visa app arrival
August 2012 (9th): visa issued!!!

Well, I'm no help! Didn't even manage to remember to ask the questions I had, let alone the eyes open/closed one! Sorry about that. And the photobooth was out of order Monday, no idea how you could go about finding out if it's back up and running before you get all the way in there.
Nice to have some relaxed restrictions on bringing electronics into the Embassy, though.

I've just been to the Embassy this week to report my baby's birth and apply for her passport. Her eyes are closed in her photo and it is perfectly acceptable x
