Pension on FBAR/FinCEN Form 114

Hello,
Over the past couple of years I started putting money into a group personal pension plan arranged by my company. It's partly employer contribution but I decided to play it safe and report it on the FinCEN Form 114. My question is how do I place it on the form? Should it be under individual accounts - like my other bank accounts? Also, under "type of account" should i check "other" and fill in "group personal pension plan" and use my policy number as the account number? Any help would be greatly appreciated.

I list mine under individual accounts and 'other'. In type of account (q16) I put 'retirement pension'. q15a is also checked. Yes, use any policy number for q18.
