Reporting change of address

Hi guys. We are moving into our new house soon, so I wanted to be prepared for reporting my change of address. I got this link, https://www.gov.uk/change-circumstances-visa-brp/youre-in-the-uk-and-have-a-brp, from a previous thread on this forum. I got a bit confused though because when you follow the links it takes you to a document that asks you to select your visa status as either Tier 1, 2, 4, or 5. As far as I am aware, spousal visas are none of these. Am I looking at the right place??
Engaged: June 2014
Married: July 30 2014
Visa Application Received in UK: Nov. 27 2014
Visa granted: Dec 12 2014
Moves to UK: Jan 30th 2015

Yes, that's the correct form: https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/300948/CoC__Migrant__04-14.pdf

For some weird reason it doesn't have spousal visa as an option and seems geared towards Tier visas, but it's the only form and is the right one for you.

Um... I moved in November, and I just did this:

Was that not right?  Do I need to do that form?
9/1/2013 - "fiancée" (marriage) visa issued
4/6/2013 - married (certificate issued same-day)
5/6/2013 - FLR(M)#1 in person -- approved!
8/1/2016 - FLR(M)#2 by post -- approved!
8/5/2018 - ILR in person -- approved!
22/11/2018 - Citizenship (online, with NDRS+JCAP) -- approved!
14/12/2018 - I became a British citizen.  :)

That form is supposed to be for people who have a visa application being processed, but they have not received their visa yet.

The page that SheIsis485 linked to above says:

You can change your address without having to apply for a new BRP.

Which form you fill in depends on whether you:

- have a valid BRP
- have applied but haven’t had a decision letter

If you have a valid BRP, it takes you to this form: https://www.gov.uk/government/publications/notification-of-change-of-circumstances-form-mcc

If you have applied for a visa but don't have a decision letter yet, it takes you to this form:
https://visa-address-update.service.gov.uk/

However, I'm really not sure if it makes a difference which form you use or not.

Hmm.  It's rather confusing, because when I filled out the form, it did want to know my BRP number, and other details that acknowledge that I have (or had) a visa already.  Now I'm worried that I failed to notify Home Office adequately.  I found my page just by searching "report change of address UKVI" in Google.  It was a *.gov.uk link, and it seemed to fit my circumstances, so I used it.  I didn't even know about the other form until this thread.

This was the confirmation email I received:

I'm finally getting around to this as things post-visa and post-move are finally settling down. I have a couple questions that the instructions don't really explain and was hoping you guys might know the answer.
1. Is the PBS reference number the BRP number?
2. The "contact details" section seems to be the only one that doesn't have a "date change becomes effective" question. Should I put the effective date at the bottom of the form at Q29?
3. Since I am on FLR(M) - what do I put for Q5, which is the one asking which migrant tier I am on? Should I put "FLRM" underneath it?
3. The directions say to list the previous and new addresses....but there is only room for one address???? :-(
