Ds 2029 reporting birth - error in form?

Ok so this form.... Ds2029 from this link.....

Page 5. When my husband tries to put start  dates in the left side of the page it keeps resetting the dates when he clicks out the box. All start dates resetting to 02/02/2013. Am i going crazy or does this happen to other people? Would someone mind trying it for me to see if its happening on your computer. Page 4 start dates work fine. Just page 5 om the left side.

I did this a couple of weeks ago and just completed by hand.  No issues.
