oh yes... 'my bad' annoys me big style.
Thankfully i only ever hear it on US tv programmes, which i dont watch very often, but... UGH!!! 
When did people start saying this, and more importantly, when will it STOP?!

HIGHLY agree... ALL my 'bad' students like to say it, and I've had to find a way not to show my annoyance, else they do it more. 
What's really irritating is they say it when I'm trying to reprimand them, so I have to hold off on the sarcasm.  ;)

The latest greeting amongst "dudes" seems to be "What's goin' on"  (yerk!)

I cringe at people ordering in restaurants saying "Can I get ..." (not as a question) and no please or thank you.  Is it just an American thing?

My parents keep telling me off for saying 'Can I get...' because it's very 'American'. I never used to say it, I grew up saying 'Please can I have...?', but then I spent a while in the US and came back saying 'Can I get...' instead (I also come out with the occasional 'gotten' and give you 'a call' instead of 'a ring'). I do always say please and thank you though :).

My son (born in the UK and moved to the US at age 14) still says "garridge" instead of "gar-ahge" after 12 years of exposure to the "bad" influences of high school and college etc.  He has picked up alot of Americanisms tho.  But he can still do Yorkshire and I never was able to get that right even after 20 years in Leeds.  Except maybe "ta, luv"  ;D

a big hate of mine is "off of", which is just plain illiterate.

I don't like either of those!  >:(

I also really dislike it when people say, "It's all good." UGH.

Oh, and when people use "lay" when they really mean "lie" as in, "I'm going to go and lay down now." AAARRRGGGH!

My bad was in the first season of South Park, so it is at least as old as 1997.  Kids are still using this? 

Were they frozen in carbonite or something?

Some people think "my bad" originated in pick-up basketball as a phrase used by young urban players when admitting to an error. It was started by the Sudanese Manute Bol, not a native English speaker, who joined the Golden State Warriors in 1988 after which several Warriors players started using the phrase.

Guilty, as charged. :-[ ;D

I'm the world's worst when it comes to that.
Example: "Where's the cat?" "He's laying on the bed." or to my son, "If you're not feeling well, why don't you go lay down?" :-[ *cringe*

I think one of the things that gets on my nerves the most is someone saying "You know what I'm saying?" or "You know what I mean?" after every sentence. Grrrrrr. >:(
