Okay, the applicant should send her actual old passport, not a copy of it with the stamps.
Also, you have quite a few things you don't need there.
Take out the following:
• Confirmation of visa application payment email
• Cover letter for the "re-application" (this should be in her cover letter - I gave you an outline for that the other day... that was both letters combined into one, just 2 paragraphs total plus the contents of the package)
• 1 Passport size picture of sponsor (pretty sure I already said you didn't need this)
• Copy of sponsor’s entry/exit stamps (you only need the bio page - don't include the stamps)
• Latest Energy /Water bills (they don't prove ownership so are irrelevant)
• Council tax statements for the last 4 years (they don't prove ownership so are irrelevant)

So, I'm just going to rearrange the documents in the order they should be presented in the package:
Applicant
• Priority visa receipt email
• Cover letter (stating type of visa applied for and refusal details, plus contents list. No personal details about your relationship - it's just a contents letter)
• Printed online Application form
• Appendix 2
• IHS Reference Number email
• Applicants old passport
• 1 Passport size picture
• TB Test
• IELTS Life skills A1
Sponsor:
• Copy of sponsor’s passport's bio page only
• Letter of support (detailing how each requirement is met)
Sponsor's Financial evidence:
• 6 months' Bank statements (should be fine if printed by the bank)
• 6 months' stamped Payslips
• Employment letter
• Accountant letter ( to address the mistake he has done in the payslips)
• P60
Accommodation documents:
• Tenancy agreement
• Landlord letter
Relationship evidence:
• Marriage booklet + its translation by a sworn translator
• Photos to prove relationship (no more than 2 or 3 photos)
• Flight tickets of our visits and trips
• Screenshots of skype/whatsapp/viber communication . Call logs
And at the back, you include photocopies of all original documents you wish to be returned to you
